While participating in the Adult Day Health Care program, you remain under the care of your personal physician who recommends available services including:
- Nursing care
- Health care monitoring of blood pressure, blood sugar levels, medications and other health regimes
- Personal care services including showers
- Occupational therapy
- Physical therapy
- Speech therapy
- Nutritional guidance (including meals and snacks)
- Therapeutic recreation
- Social service counseling / case management / support groups
Transportation to and from the registrants’ home and the Schofield Adult Day Health Care Program may be arranged.
Specialized services can be provided on-site and in the community if needed:
- Ophthalmology
- Audiology
- Laboratory services
- Medication reviews
- Dentistry
- Radiology
- Podiatry
- Durable medical equipment
