Specialized, Holistic Care for Patients with TBIs

Because of the unique nature of brain injuries, programs designed to help must address the cognitive, physical and emotional aspects of the program participant. Schofield offers services for patients who are living with the effects of a Traumatic Brain Injury:

Traumatic Brain Injury Waiver Services from our Certified Home Care Program

Our professional staff will help to identify daily living needs and respond to those needs by arranging for services. We promote activities to increase independence and ease re-entry into the community. In addition, we help increase the individual’s productivity in meaningful activities. Home and Community Support Services (HCSS)

Schofield’s HCSS aides provide non-medical daily living supports to TBI program participants. They prompt, supervise and train individuals with TBI in bathing, personal hygiene, dressing, meal preparation, eating, household tasks, laundry, shopping and socialization. Transportation services may be provided if that is part of the participant’s plan of care. Schofield Home Health Services works closely with the TBI Service Coordinator to help the participant achieve their goals. Schofield’s Adult Day Health Care Program

Adult day health care is invaluable to individuals seeking alternatives to nursing home placement. The program provides physical, occupational and speech therapies, nursing care, personal care, case management, nutritious meals and snacks, health monitoring, recreational and other services in a supportive group environment.
