Currently we are looking to hire part time or full time service coordinators to service individuals that have suffered a traumatic brain injury. Service coordinators will help in developing, implementing, and monitoring service plans. Service coordinators will help participants increase independence and become more integrated into the community.   Service Coordinators will advocate for the participant and link participant with appropriate resources to assist in meeting goals and living safely in the community.

 

Qualifications:

*Master Degree in Social Work, Certified Rehabilitation Counselor, Certified Special Education Teacher, or other related human service degree with one year experience providing case management to individuals with disabilities.

Or

*Bachelor’s Degree and two years experience in providing case management to individuals with disabilities.

Or

*Associates Degree and three years experience providing case management to individuals with disabilities.

 

Applicant must be able to work as a team, be self-directing, and able to multi task.
