The Social Worker is responsible for specific duties as assigned to him by the Coordinator of Social Services. This includes, but is not limited to, assessing and meeting the psychosocial needs of the residents and family intervention as needed, assisting with Medicare and Medicaid procedures and working as part of an interdisciplinary team, as well as, other requirements.
- Education and Training:A minimum of a baccalaureate degree, from a recognized college or university, in social work or related field. A Masters candidate in social work may be considered. This person must be willing to keep informed of legislative changes and programs affecting the fields of Social Services, Gerontology, and Long Term Care, and attend appropriate programs.
- Experience:Experience in geriatrics is preferred in institutional and/or home care setting.
Job Duties and Responsibilities (includes but not limited to):
- Implements a plan of care that meets the emotional and social and psychological needs of each resident assigned.
- Provides counseling service for the resident and/or family members maintaining confidentiality of information.
- Acts as an advocate for the resident and their family as well as serve as a liaison between the facility and the families of perspective residents.
- Arranges for admission of new residents in conjunction with administration and the Admission Assistant.
- Acts as a liaison between financial assistance agencies, insurance carriers, residents and families. Assists in determining resident eligibility for financial aid and assists the resident and family with completion of placement necessary to secure financial assistance.
