Admission Information:
We welcome your questions regarding admission to the Schofield Residence. Please call to arrange a tour of our facility. To schedule a tour, call our Admissions Assistant.

A formal admission application and a Patient Review Instrument (PRI) must be completed and approved before admission to the Schofield Residence can take place. Our Admissions Assistant will be happy to discuss the admission procedure with you and answer your questions.

Amenities:
The Schofield Residence is located on a beautifully landscaped five-acre site at 3333 Elmwood Avenue in the Town of Tonawanda. Our nursing facility residents enjoy a safe, residential neighborhood, with convenient access to major roads and highways for easy visitation. In the warm weather months, residents enjoy visits in our rose garden, at the front of our building, and in our outdoor patios.

Upon visiting, you will agree that the Schofield Residence has a warm, family atmosphere. Our beautiful atrium, filled with plants and flowers, brings nature indoors. Plants and artwork throughout the building make you feel at home.

Because we realize the important role that pets play in many lives, family pets are always welcome visitors.

We offer private and semi-private rooms on two floors. Each resident room is equipped with a nurse call system, handicapped accessible bathrooms, state-of-the-art beds and new furniture.

Upon entering the Schofield Residence, many of our visitors comment on the cleanliness of our facility. Our housekeeping staff members work very hard to maintain our reputation for cleanliness. At the Schofield Residence, we know that excellent housekeeping enhances the living environment for all our nursing facility residents.

Health Care Services:
Staff physicians provide medical services, assuring continuity of care. Our Medical Director, Dr. Robert Stall, is a licensed geriatrician specializing in the medical needs of the older adult.

The Schofield Residence has earned a reputation for excellence in nursing care due to our highly qualified and experienced nursing team. Registered nurses, licensed practical nurses and certified nurses' aides provide nursing care.

We are committed to maintaining our staff at appropriate levels. Our nurse staffing levels are consistently above New York State requirements. Registered Nurses are on duty 24 hours a day, seven days a week. We have low turnover within our licensed nursing staff, and because the Schofield Residence encourages continuing education, many of our nurses hold bachelor's degrees.

Skilled Nursing Services that are available in our Nursing Facility include, but are not limited to, the following:
Intravenous hydration and IV antibiotic therapy
Oxygen Therapy
Monitoring/assessment of acute medical conditions
Wound care (surgical, stasis ulcers and pressure ulcers)
Naso-Gastric (NG) or Gastrostomy (G) tube feedings
Care for the terminally ill
Intake/output monitoring

Coordination of Care:
Each resident's health care needs are under the direct supervision of a registered nurse manager.
Residents and their families are encouraged to attend Resident Care Meetings, an interdisciplinary team meeting, to review and make comments on their care plan.

Rehabilitation Therapies:
Our Rehabilitation department provides services five days a week based on the recommendations of each resident's physician. Many people, who are medically stable, but not ready to go home, come to the Schofield Residence for short-term rehabilitation therapy.

Physical therapy, occupational therapy, speech-language pathology and audiology are provided on-site. The goal of therapy is to restore or maintain residents' highest functional abilities to enhance dignity and comfort while promoting independence.

The Nursing department provides ambulation and range of motion that complements the resident's therapy program and recommendations.

Specialized Services:
Specialized health-care services that can be provided on-site include podiatry, dentistry and ophthalmology. Other on-site consultants include, neurology, psychiatry, surgery and urology. For the resident and their family, this eliminates many trips to physicians outside the facility.

Dietary Services:
The Dietary department prides itself on personalized nutritional care provided by Registered Diet Technicians. We use a liberalized diet manual, which eliminates many of the restrictions seen on diets of the past. What this means is that the resident receives the best of both worlds: a diet that meets both their nutritional needs and personal preference.

Varieties of diet textures are offered which enables us to provide each resident with the optimum texture they can tolerate. This allows us to adjust many food items so residents may continue to enjoy the foods with which they are familiar.

Recreation:
Schofield's Activities department provides a wide range of programs for our residents. Programs run throughout the day and several evenings per week. We also have a full day of activities on Saturday and Sunday. Our Activities department arranges non-denominational and Catholic religious services on a regular basis.

A Certified Therapeutic Recreation Specialist heads the Activities department. With multiple full-time and part-time staff members, Schofield's Activities department staffing is far above state mandates.

Family and friends are welcomed and encouraged to visit residents and participate in our Activities programming.

Social Services:
Our Social Services department is here to help residents and their families with social, emotional and psychological needs and concerns. They also help with Medicare and Medicaid questions, resident orientation to our nursing facility, resident transfers and discharge planning.

Payment for Services
Schofield Residence participates in the Medicare and Medicaid programs. Private insurance may also cover some of our services. Please call us for more information.
