With health-care costs rising rapidly, the Schofield Residence Long Term Home Health Care Program offers a cost-effective way for chronically ill or disabled individuals to receive comprehensive health-care services in the comfort and security of their own homes.
In operation since June 1985, the Schofield Residence Long Term Home Health Care Program serves patients in Erie County.
How the Program Works:
The hallmark of the Long Term Home Health Care Program is the coordination of care provided by Schofield's dedicated community health nurses. A nurse case manager develops a plan of to meet each patient's individual needs. The community health nurse then monitors the patient's care, provides direct home health care services and makes arrangements for other services to be provided.

- Services Offered:
- The LTHHCP provides, coordinates and manages the delivery of numerous health care services including:
- Nursing care
- Physical therapy
- Occupational therapy
- Audiology
- Speech therapy
- Home Health Care Aide services
- Personal Care Aide service
- Medical Social services
- Nutritional guidance and counseling
- Homemaker and Housekeeper services
- Medical Supplies and Equipment
- The program can also arrange for:
- Personal emergency response system
- Meals on Wheels
- Respite care
- Adult day health care
- Social day care
- Visits by specialists such as podiatrists, dentists and ophthalmologists are arranged as needed.
- Specialized Services:
- AIDS Home Care Program:
- Since 1993, the Schofield Residence LTHHCP has served patients with HIV infection and AIDS through the AIDS Home Care Program. The Schofield Residence participates in the HIV Home Care Uninsured Fund administered by the AIDS Institute, NYS Department of Health. This fund provides reimbursement for some health care services for medically and chronically dependent HIV positive ill persons who are uninsured or underinsured.
- Traumatic Brain Injury Program
- The Schofield Residence Long Term Home Health Care Program received approval for a Home and Community-based services Medicaid Waiver for individuals with Traumatic Brain Injury (HCBS/TBI) in 1995. The program serves residents of Erie County who have been diagnosed with traumatic brain injury. This goal of this program is to meet the needs of individuals with traumatic brain injury who require nursing home level care, but who prefer to live in their homes or private residences. - Private health insurance, Medicare, Medicaid and private payment are accepted. Call our staff for more information.
- Admission Criteria:
- To receive services from Schofield's LTHHCP an applicant must:
- 1. Be an Erie County resident;
- 2. Be medically eligible for the services of a hospital or residential health care facility;
- 3. Be Medicaid eligible, have Medicare or private insurance coverage, or be willing to pay for services privately.
- Arranging For Services:
- Patients enter the program upon discharge from a hospital or nursing home, or may apply directly from their own home. Our staff is happy to assess any individuals for possible placement. Please call us with questions regarding our services, our admission procedures or to refer a patient.
