Service Coordination
The Schofield Residence Long Term Home Health Care Program is available to help meet the needs of individuals with traumatic brain injury. This program is an excellent alternative to nursing facility care.
Service Coordination-Our professional staff will help to identify daily living needs and respond to those needs by arranging for services. We promote activities to increase independence and ease re-entry into the community. In addition, we help increase the individual's productivity in meaningful activities.
The Schofield Residence Long Term Home Health Care Program received approval for a Home and Community-based services Medicaid Waiver for individuals with Traumatic Brain Injury (HCBS/TBI) in 1985.
To be eligible for the waivered services and other services offered through Schofield's Long Term Home Health Care Program individuals must:
Be diagnosed with traumatic brain injury
- Be a resident of Erie County
- Choose to live in the community
- Be enrolled in the Medicaid program.
