Home and Community Support Services

Schofield Home Health Care Services offers Home and Community Support Services (HCSS) Aides to provide daily living supports to TBI program participants. The goal of this program is to meet the needs of individuals with traumatic brain injury who require nursing home level care, but who prefer to live in their homes or private residences.

Home and Community Support Services are non-medical. Services include supervision, companion and chore services. We also provide transportation services for the participant if that is part of their plan of care. Transportation allows the participant to use non-medical services in the community in order to improve the individual's ability to make use of needed services, and to improve the individual's integration in the community.

How does the program work?

First a service plan is developed by the TBI Service Coordinator of the participant's choice. We work closely with the TBI Service Coordinator to help the participant to achieve their goals. The TBI Service Coordinator and participant sets goals, our HCSS Supervisor then prepares a report for the HCSS Aide to follow. The Schofield HCSS Aide then comes to the participant's home in the community and is available to prompt, supervise and train in some of the following areas:
