Schofield Home Health Care Services is a licensed home health care agency. We provide a wide range of services in the home to meet your daily needs. Services are available up to 24 hours daily, seven days a week, for people of all ages. Call us for a free needs assessment.

How the program works

A Registered Nurse (RN) will come to your home to develop a plan of care that will meet your individual needs. The nurse then coordinates and monitors your care. Home health aides, personal care aides, homemakers and housekeepers are available to provide the hands on help in your home.
