We are currently hiring for the following position(s) at Schofield Residence Long Term Home Health Care Program (LTHHCP) with offices located at 2757 Elmwood Avenue in Kenmore NY:
Home Health Care RN for Schofield Residence LTHHCP:We are hiring a Home Health Care RN to perform comprehensive assessments of physical, emotional, psychosocial and environmental needs of the patients. What does a Community Health Nurse do?Community Health Nurses coordinate all the care provided to our patients. They develop a care plan to meet each patient's individual needs, and then monitor the patient's care, provide direct home health care services and make arrangements for other services to be provided as necessary.
