HIV/AIDS Home Care 

Overview

Since 1993, the Schofield Residence Long Term Home Health Care Program has served patients who are living with HIV infection and AIDS through the AIDS Home Care Program. We are the only program of its type in Erie County. We believe that by providing comprehensive, coordinated health care services we can greatly enhance the patient's self-reliance and quality of life.

The program is designed to allow patients who are medically eligible for Skilled Nursing and Intermediate Care Facilities to be cared for at home. The care of each patient is developed and monitored by a community health nurse. The nurse provides direct home health care services and makes arrangements for other services to be provided as necessary.

Because the needs of individuals are not static, flexibility is built into our system that permits responding to the individual's changing needs with the most appropriate services and care.

A vast array of services can be arranged as part of the AIDS Home Care Program. Some of these are:
