Admissions Information

Please contact us at administration@schofieldcare.org or fill out the form below for more information on the program and to arrange a tour. Applications for admission can be obtained by calling the Schofield Adult Day Health Care Program at 716-819-9200, or download an application here.

A current medical exam and report is required from the individual's primary care physician before a Registered Nurse makes a home visit to assess the registrant for eligibility.
