8:00 a.m. to 1:00 p.m.

Located on the third floor of Sheehan Memorial Hospital, the Schofield Adult Day Health Care Program offers a cost effective community-based health and rehabilitative program that provides all the services of a nursing home in a day setting. Registrants must attend a minimum of five hours a day, one day a week.

The program gives individuals and their families an appropriate alternative to institutional placement. It also reduces the incidence of physician emergency room visits and hospitalizations while providing education, support and respite for family members who are caregivers.

Schofield's Adult Day Health Care Program is a Medical-Model Program.

The Benefits
  • Adult day health care enables people to receive the care they need in a community setting. This can prevent or delay institutional placement.
  • A coordinated program of medical services, including restorative and maintenance therapy as well as socialization and preventive health care, is provided in a single setting.
  • Individuals who might otherwise be isolated receive an enriching educational and social experience.
  • The program provides family members' relief from the responsibilities of caregiving.

Who Benefits From This Program?
Any individual, who has medical/health and/or rehabilitation needs and living in the community, can benefit from this program. The program serves Erie and Niagara County residents. This includes individuals living in senior housing, assisted living and enriched housing.

Common diagnoses for people benefiting from the program include:
Strokes
Joint replacements
Fractures
Amputations
Arthritis
Parkinson's disease
Diabetes
Multiple Sclerosis
Traumatic Brain Injury

Schofield's Adult Day Health Care Services include:
Nursing care
Health care monitoring of blood pressure, blood sugar levels, medications and other health regimes
Personal care services
Occupational therapy
Physical therapy
Speech therapy
Audiology
Nutritional guidance
Therapeutic recreation
Social work

Transportation, to and from the registrants' home and the Schofield Adult Day Health Cared Program will be arranged.
